What are the COVID precautions?
If you have experienced symptoms of COVID, tested positive, or exposed to a person with COVID in the past 5 days, please call our office to reschedule your appointment.  Upcoming surgeries may require a negative COVID test prior to the procedure.  Please visit our COVID updates page for additional information.

Do I need a referral to see a Tri-State Orthopaedics provider? 
Our office does not require referrals; however, a referral may be a requirement of your insurance plan. If you are unsure if your plan requires a referral, please contact your insurance company. 

How can I make an appointment?
There are 3 ways you can request an appointment.

1.  Call our main office – 812-477-1558.  For the Vincennes office call – 812-882-6637.
2. Request an appointment on our website.
3.  For those that have a TSOS patient portal account, request an appointment through your account.

What should I bring to my appointment? 
Photo ID, insurance card, co-pay and a list of current medications

Will I have a co-pay?
Co-payment depends on your specific insurance plan and benefits.  Patients are responsible for understanding their deductible status and network benefits.  If you have questions about your benefits, please contact your insurance provider.  We will collect any co-payments due at the time of check-in.

Which doctors work at the different offices? 
Each of the doctors’ locations are posted on our website on our Team page.

What if I have a question for the physician after hours?
If you are experiencing a medical emergency, please call 911. If you have non-urgent questions, please call our office to leave a message with the answering service or submit a question through our online patient portal.  Questions through the patient portal will be addressed the next BUSINESS day.

Do we have wheelchairs available for patients?
Yes, you can request to use a wheelchair at our front lobby info desk.

Where will I go to have my surgery? 
Our doctors perform majority of their surgeries at the hospitals below:

Do you offer financial assistance?
Yes, please complete our financial assistance application and return to our office by mail (225 Crosslake Dr., Evansville, IN 47715) or drop off at our info desk.

Do you accept my insurance plan?
We accept many insurance plans. Please call your insurance company to verify if we participate with your specific plan type.

How do I make a payment or access my bill online?

I have a question on a statement/balance who do I call?

How can I obtain medical records?
Submit a request including a completed authorization to release medical records form via our patient portal, email medicalrecords@tristate-ortho.com or contact 812-477-1558 (Option 6).

What if I have medical documents that need to be completed?
Documents may be submitted to our Medical Records department via fax 812-476-6867 or email medicalrecords@tristate-ortho.com.

How do I schedule an appointment with a provider regarding a possible sports concussion?
There are several providers in the tri-state area involved in the Ascension St. Vincent Concussion Alliance that evaluate concussions for athletes.  Please call 812-403-DING (3464) to request an appointment with one of these providers.
